Please print the relevant form, fill it out & bring it with you to your first office visit. Although it appears lengthy, it can be filled out in less than 30 minutes. It is very helpful to complete this form prior to coming to your appointment.

A few things to know about our clinic!
Office Hours: The office is open Monday noon-4pm and by appointment. Tues-Wed-Thurs hours are from 10:00-6:00pm. Friday the office is closed.
Appointments can be made by phone @ 401-455-0546 or in person. Should you reach the answering machine, please leave your name & number. We will get back to you as soon as possible.
Payment: All patients are responsible for payment of their service and medicinal charges. Payment is expected at the time of service. For your convenience we do accept Mastercard/Visa.
Emergency: Please note we are NOT an emergency medical clinic; we do not treat acute emergency care. Please contact your nearest emergency room or dial 911. Once evaluated by a medical doctor, should you wish to choose alternative or complementary options for your condition, please call for an appointment. Dr. Frodermann will discuss this option with you.
